Subcostal incision hernia after open cholecystectomy. Incisional hernia repair twice one open and one laparoscopic repair had performed but hernia recurred. We repaired the hernia with the hybrid laparoscopic IPOM technique. The hernia type is Swiss cheese and small bowels are dens attached to the previous mesh. The small bowel is mobilized through the small subcostal incision. After the incision closed, the abdomen insufflated again and the 25x20 cm IPOM mesh covered the whole hernia side The mesh was fixed with four transfascial sutures and tacks.
